Dear Prospective Investor,

Thank you for your interest in becoming a partner.   On this page you will find a link to a single MS Word Document that contains the ITC Operating Agreement and Subscription Agreement.   To properly execute this two documents, you will need to fill out and sign page 27 and page 32 in front of a Public Notary, having the Notary sign and emboss the same.  Also, on pages 28 and 29 there is a section requiring  you to designate your status as an investor.  You will need to initial the proper section according to you status as an 'Accredited Investor' or as a private party.

The final step is to send a set of signed and notarized originals to the office along with your investment check.
